Enter your first name, middle initial and last name in these 3 fields.

Remember an email address will have an '@'. Notice also the red '*'. This indicates that this is a required field.
Try to type a letter in any of these 3 fields. Only numbers are allowed and only 3 or 4 numbers can be entered.
Remember in a group of checkboxs more than one can be selected
